The horror sequel moves the story to Akishima Island, where a YouTube ghost-hunting team called Dead True investigates the abandoned Blackwood Sanitorium and uncovers real supernatural threats. Player choices will determine who lives and dies, continuing the series' signature branching-story design.

The sequel follows the Dead True crew as they investigate Akishima Island, a remote 1970s tropical resort that connects to events from 12 years earlier at Blackwood Sanitorium. The team expects to film another ghost-hunting episode, but the investigation uncovers actual paranormal threats—including the return of Wendigos. Geek Tyrant reports the cast includes Dacre Montgomery as Owen, the show's cocky host, and Sarah Catherine Hook as Izzy, a new intern joining the crew.
Other cast members include Gia Kim as Yoona, a survivor bearing a dark scar who warns the team of supernatural danger, and Spence Moore II as Nate. Peter Stormare returns as Dr. Hill, the uncanny psychiatrist from the original game. Neil Newbon joins the ensemble cast in an unannounced role, rounding out the performance lineup.
AdHoc Studio's involvement signals strong narrative credentials for Until Dawn 2. The studio created Dispatch, an acclaimed game known for sharp writing and dynamic character relationships.
