Key Points
1
Indictments describe Titi transmitting detailed hospital-readiness intelligence—such as patient counts and the underground relocation of patients from Carmel Hospital to Rambam Health Care Campus—compiled during both routine operations and missile-attacked periods at Rambam and Galilee Medical Center.
2
During a visit by President Isaac Herzog and Finance Minister Bezalel Smotrich to the Galilee Medical Center in Nahariya, Titi allegedly alerted his Iranian handler that the officials were on site and photographed Herzog and members of the president’s entourage.
3
Authorities say his spying extended beyond hospitals to civilian and political targets, including Hostages Square in Tel Aviv, Haifa Museum security cameras, and anti-war placards at protests in Haifa and Tel Aviv; prosecutors note payments of up to 10,000 NIS per assignment and a stated willingness to carry out an assault for 50,000 NIS.
4
Recruitment reportedly began via an Instagram account purporting to belong to National Security Minister Itamar Ben-Gvir; Titi sought help obtaining a gun license and gradually was drawn in, though he later indicated he suspected the contact could be an enemy party rather than the minister himself.
5
Titi was arrested on June 29 by undercover Border Police; the indictment and reporting identify the foreign handler by codename 'TOM' as part of the operation coordinated by Shin Bet and Israel Police, with the Haifa District Court handling the case.
An Israeli ambulance driver has been indicted on charges of spying for Iran, using his hospital access to photograph senior officials and relay military intelligence to a foreign handler. Times of Israel reports that Amir Hisham Muhammad Titi, 34, was arrested June 29 by undercover Border Police and charged in Haifa District Court with assisting the enemy and transmitting information to Iran-linked operatives.
Titi allegedly photographed President Isaac Herzog and Finance Minister Bezalel Smotrich during a visit to Galilee Medical Center in Nahariya. He then tipped off his Iranian handler that the officials were on site, according to World Israel News.
How an Instagram Account Kicked Off the Spy Recruitment
Titi's path into espionage began with a fake Instagram account. JFeed reports the account pretended to belong to National Security Minister Itamar Ben-Gvir. Titi reached out seeking help getting a gun license. Handlers on the other end then slowly drew him in over time.
Titi later told investigators he suspected the contact might not really be Ben-Gvir. Still, he kept working with the handler, who used the codename 'TOM.' Times of Israel notes the recruitment began around October 2024 and escalated from there into active intelligence gathering.
Hospital Secrets Passed to Iran During Missile Attacks
Titi used his ambulance job to gather detailed medical intelligence. He documented patient counts, photographed hospital facilities, and tracked security arrangements at Rambam Health Care Campus and Galilee Medical Center. He also reported the underground transfer of patients from Carmel Hospital to Rambam during missile-attack periods, according to Yahoo News.
That kind of information has direct military value. Knowing how many patients are in a hospital — and where they are moved during strikes — tells an enemy how facilities function under attack. The Shin Bet and Israel Police coordinated the investigation that uncovered this pattern, says Caspian Post.
Targets Ranged from Protests to Security Cameras
Titi's spying went well beyond hospitals. Prosecutors say he also photographed Hostages Square in Tel Aviv, security cameras at the Haifa Museum, and anti-war signs at protests in both Haifa and Tel Aviv. Each assignment reportedly came with a payment of up to 10,000 NIS, according to Times of Israel.
The indictment also says Titi expressed willingness to carry out a physical assault for 50,000 NIS. World Israel News reports that prosecutors included this as evidence of how far his handlers were pushing the operation, and how far he was willing to go.
Arrest, Charges, and What Comes Next
Border Police undercover officers made the arrest on June 29. The Haifa District Court received the indictment, which charges Titi under Israeli laws on assisting an enemy and passing information to a foreign power. JFeed confirms the Shin Bet and Israel Police worked together on the case from the start.
The case is one of several recent Israeli prosecutions tied to Iran-linked recruitment networks. It highlights a broader pattern: handlers target ordinary workers with routine access to sensitive places. An ambulance driver can go anywhere. That access, prosecutors argue, made Titi especially useful to Iran.
